FAQ

What is the difference between the Healer and the Surgeon in Chained Beasts?
Official store text names a Surgeon between acts who treats afflictions or offers augmentations and amputations. Gameplay video and community guides show a Night Healer screen with potion-style affliction cards. Featherweight has not stated whether they are the same NPC or separate systems—we treat them as related but unconfirmed roles.
How do Dead Weight and Ultra Hydrated work on a run?
Dead Weight is the official achievement "Amputate a body part," tied to Surgeon surgery choices. Ultra Hydrated is "Win a run while only ever drinking water" at the Healer—typically picking Water when you meet its health requirement instead of affliction potions.
How do you unlock Turning the Tide against the Flail Master?
Steam lists Turning the Tide as "Send the Flail Master flying with his own flail." Featherweight has not published step-by-step boss mechanics; the achievement text implies redirecting his flail attack back into him. His act or round index and HP are not disclosed in launch materials.
What are The Senate and Rebel Leader in Act 3 Rome?
Rebel Leader requires you to "Reach The Senate." Steam's internal API name for that achievement is CompleteAct2, following CompleteAct0 (Carthage) and CompleteAct1 (Germania)—so we infer The Senate is the third-act completion milestone in Rome, not a separate minigame name on screen.
What counts toward Medical Marvel's 20-skull clear?
Medical Marvel is "Win a run while suffering afflictions totalling at least 20 skulls." Affliction cards show skull severity slots on official UI; you must still win the run with that combined total active—often by stacking high-tier Healer potions and Trainer misery rather than curing everything.

How do Healer potions and the Surgeon shape your endgame run?

Between arena days, Chained Beasts pushes you through night camps where afflictions stack and economy choices matter for the Rome finale. This guide focuses on Night Healer potion menus, the between-act Surgeon described in official store copy, the Flail Master achievement fight, and reaching The Senate for Rebel Leader—without inventing HP tables or undisclosed boss phases.

For skull mechanics and Trainer screens, see the afflictions and Trainer guide. Merchant passives that interact with healing and chains are on the merchant items page. Boss order and achievement hooks sit in the all bosses hub.

These six frames are matched to timestamps in community guides so you can jump to the same UI we reference. Numeric combat stats and undisclosed boss data are still omitted—only on-screen text and official achievement wording.

  1. 1 Read skull severity on Night 1 Healer potions

    05:45 ↗

On Night 1 the Healer offers five cards: Punctured Lung III (67g, quickly run out of breath), Weak Shoulders II (47g, throw distance greatly decreased), Near Sighted II (45g, blurry vision), Missing Toe I (21g, sometimes trip when moving), and Water (requires 25+ HP). Higher Roman numerals and gold costs usually mean heavier misery—plan around Medical Marvel or avoid stacking if you want a cleaner win.

  2. 2 Buy Merchant passives before the next act

    10:00 ↗

The Night Merchant row includes A Gift for the Doctor (Passive, 60g, +100% healing from potions), Chain Upgrade (Arena, 90g, longer chain and increased trip damage), Obsidian Pilum (50g), Flaming Obsidian Sword (70g), Medicine (50g), Quilted Armor (40g), Oath Torc (40g, shares equipment stats), and Armor Plate (60g, negates first 3 hits). Doctor plus Medicine synergizes with Healer-heavy runs; Chain Upgrade helps chain trips in boss arenas.

  3. 3 Compare alternate Merchant loadouts on misery nights

    01:52 ↗

A different Merchant roll highlights A Gift for the Quartermaster (65g, more arena weapons), Poisoned Apples (90g, half the enemies start hurt), Explosive Bow (50g), Obsidian Hammer (60g, sends enemies flying), Medicine, Sword Guard, Shield Spikes, and Leather Strop (+100% crit damage). None of these replace Healer potions, but they change how much punishment you can afford before Rome.

  4. 4 Level beasts at the Night II Trainer before act bosses

    02:22 ↗

The Night II Trainer screen reads "Time for some reps." A blue horned gladiator beast gains +5% HP before the next act's boss battles. Trainer XP still feeds achievements like Untrained and Overtrained—see the afflictions guide for LEVEL bar details. This night is your last comfortable stat bump before Flail Master–tier checks and the Praefectus siege.

  5. 5 Rest at the Day IV camp before the iron gate

    06:30 ↗

Day IV shows four gladiator beasts resting around a glowing campfire inside a dark stone chamber, with an iron portcullis gate ahead. Camp nights are when Healer, Merchant, and Trainer choices stack into your Medical Marvel or Ultra Hydrated plan—bring enough gold from [crowd scoring](/guides/crowd-favorite-and-scoring/) if you intend to buy out key passives.

  6. 6 Fight around arena pillars with chain tether pressure

    03:08 ↗

A blue horned beast and human gladiator tethered by a heavy iron chain fight around a central stone pillar and broken clay pot in a blood-stained sand arena. Chain geometry like this shows up in Goliath-style set pieces and late Rome fights—practice trips and throws from the [controls and chain combat guide](/guides/controls-and-chain-combat-guide/) before Flail Master and Praefectus.

Healer potions vs between-act Surgeon — what do official sources say?

Healer (night camps): Verified footage shows a HEALER NPC offering potion cards that apply or trade afflictions, each with skull-rated severity and gold costs—plus Water with a health gate (as shown in the Night 1 frame above: requires 25+ HP). Store text adds that potions can relieve pain but side effects may cause worse afflictions, and Twitch integration can let viewers vote on which potion you drink between rounds.

Surgeon (between acts): Steam store copy states that a Surgeon between acts can treat afflictions or offer augmentations and amputations in exchange for new skills, with a cost on every surgery. The Dead Weight achievement (Amputate a body part) aligns with amputation choices—not with drinking Water at the Healer.

Ultra Hydrated (Win a run while only ever drinking water) is a Healer challenge: pick Water whenever you shop affliction relief and never take the misery potions. Medical Marvel (Win a run while suffering afflictions totalling at least 20 skulls) is the opposite extreme—stack high-tier potions and Trainer cards until skull slots sum to twenty, then still win the run.

Full achievement wording lives on the achievements guide. Difficulty modifiers that change misery cadence are summarized under difficulty levels.

Flail Master and Turning the Tide — what is disclosed?

Steam names a boss encounter Flail Master only through achievements:

Achievement Official requirement Global unlock (2026-10-16 snapshot)
Turning the Tide Send the Flail Master flying with his own flail 6.8%
Whirling Dervish Spin the flail for at least 10 seconds without stopping 2.0%

Turning the Tide is the headline Flail Master challenge: you must launch the boss with his own flail, not merely kill him with chain trips. Whirling Dervish is a separate flail proficiency check—any flail user can pursue it, but it pairs naturally with Flail Master fights where flails are in play.

Featherweight’s public announcements do not publish numeric HP, exact act index, or round number for Flail Master. The all bosses page keeps him in an achievement-backed row without inventing phases. Gameplay trailers show a large enemy swinging a flail without on-screen boss name text—we do not treat that footage as a confirmed Flail Master reveal.

Community footage: camp nights and arena chains

For another Rome-era boss with documented siege mechanics, read Praefectus and the Praetorian Guard. For a confirmed leap-attack achievement boss, see Goliath.

The Senate and Rebel Leader — Act 3 Rome finale

Rebel Leader reads Reach The Senate. In Steam’s achievement API the internal name is CompleteAct2, following CompleteAct0 (Carthage survival — Can’t Stand The Heat) and CompleteAct1 (Germania — Out of the Cold). Our inference (not spelled out in store text): clearing The Senate is the third-act completion gate in Rome, after you survive earlier acts and push through late bosses.

Official marketing shows Roman architecture and legionaries in cutscenes; SPQR appears on building pediments in trailer frames. Those shots do not display the words “The Senate” on screen—only the achievement name ties the destination to the finale.

The Praefectus and the Praetorian Guard siege (boss page) sits in the Rome endgame arc with achievements Clearing the Skies and Don’t Look Up around catapult fire. Treat Praefectus as the structured siege checkpoint before the inferred Senate milestone—not as a substitute for Rebel Leader itself.

When you are ready to track every requirement in one place, use the achievements guide checklist alongside this page’s Healer and Surgeon planning.
